I apply to be admitted to participate in the Sportskey Testing Regime for the following areas which I tick: Speed, Power, Lateral movement, Throwing technique, Throwing power, Motor proficiency and balance, Anticipation, Decision making, Visual skill, Hand eye coordination, Anthropometric data.
I accept and agree that photo and video and other images of me participating in this testing regime will/might be shown on the Sportskey website, as well as other social media platforms aimed at promoting and advertising Sportskey.
General testing results, at the election of Sportskey, will be displayed on the Sportskey website and at Sportskey's discretion, shared with other sporting bodies and institutions, such as schools and Universities [Individual testing results will only be shared with third parties if direct consent is given by the parent or guardian of the athlete].
In consideration of being allowed to participate in this testing regime I agree unconditionally to do so at my own risk and I accept full and sole responsibility for my safety and for my personal possessions in and as a result of that participation.
I acknowledge that participating in this testing regime may be dangerous to the extent that in some circumstances I may endanger my health or the health of others.
I also agree that, on being accepted to participate in this resting regime. If I am under the age of 18 years, one of my parents have consented on my behalf and that it binds both me and my parents.
I state that I have chosen freely to participate in this testing regime.
I agree to comply fully with all Sportskey's policies in relation to the testing regime and otherwise.
Before starting to participate in the testing regime I have disclosed to Sportskey any injuries, disabilities, diseases or ailments or other health concerns or dietary requirements (health concerns) which may affect my safe participation or ability to safely take part in the testing regime.
I agree to immediately notify Sportskey on the occurrence of any change in my health in my health concerns or on my development of any health concerns.
I also agree to abide fully with any decision Sportskey may make in relation to my continued participation or non-participation in the testing regime.
I release, indemnify, continue to indemnify and hold harmless Sportskey, each of its directors, staff, players, members, servants and agents from all and any responsibility or liability for any loss, damages, claims, actions, costs, expenses or liabilities whatsoever suffered or incurred by me arising out of my participation in the testing regime.
I do not and will not hold Sportskey or any of its directors, staff, players, members, servants or agents legally or otherwise responsible for injuries or loss of any nature which I suffer while participating in the testing regime as I accept fully the risk of those occurring.
I undertake not to sue or bring any claim against Sportskey or any of its directors, staff, players, members, servants or agents for any claims, costs, damages or other liabilities I may have or they may have ARISING IN ANY WAY FROM any injury suffered by me as a direct or indirect result of my participation in the testing regime as I accept fully the risk of those occurring and I acknowledge and agree that this waiver represents a full legal release and discharge of legal responsibility to Sportskey and to each of its directors, staff, players, members, servants and agents.
This waiver is provided in exchange for the goods and services provided to me by them including their permission for me to participate in the testing regime.
I consent to Sportskey and other third parties associated with the testing regime or identified by Sportskey using my name, likeness, image and/or voice ( including photograph, video, film and/or recording of the same) in any media or otherwise for an unlimited period without remuneration to me for the purpose of promoting Sportskey and the testing regime ( including any outcome).
I ACKNOWLEDGE that I participate in the testing regime of my own free will and that I have read and fully understood the conditions above before starting to do so.
